How Has the JCI Survey Changed in the 8th Edition?

With the revision to the JCI 8th Edition, the survey style has shifted significantly toward a more field-focused approachthan ever before.

Current surveys consist of two main components: tracer activities conducted in clinical settings and presentations and discussions held in conference rooms. One notable change is that the proportion of tracer activities has clearly increased.

What Are Tracer Activities?

During tracer activities, surveyors enter actual clinical settings and ask questions directly to staff members while conducting discussions based on patient care processes and medical records.

In other words, evaluation is no longer limited to whether documentation is properly prepared. Instead, it now assesses:

  • whether safe medical care is actually being provided

  • whether staff understand and are able to implement it in practice

Changes Required for Healthcare Organizations

For hospitals, these tracer activities can represent a significant burden.

This is because the staff subject to evaluation cannot be identified in advance, requiring education and preparation for all staff members.

Furthermore, in the 8th Edition, the time spent on on-site tracing itself has increased, requiring a higher level of real-time responsiveness than before.

However, this change is not simply a “tightening of the survey.”

Through enhanced staff education, it ultimately leads to improvements in the quality of care and patient safety, making it a highly meaningful direction.

If understanding and practice are embedded in the clinical setting, it improves not only survey readiness but also the safety of daily medical care.

Key Points for Achieving Accreditation

To pass the survey, the quality of education and the degree to which it is embedded have become more important than ever.

It is not enough to simply share knowledge. Education that includes clinical thinking—such as:

  • why it is done

  • how decisions are made

—is essential.

In responding to the 8th Edition, the following two points are important:

  • Structuring educational content systematically in accordance with JCI standards

  • Implementing education efficiently in a way that can be reproduced in the clinical setting

These two points can be considered key factors for achieving accreditation.

We hope that this opportunity for re-accreditation will serve as a valuable opportunity to review hospital operations and organizational culture.
